Your Medicare cardThe red, white and blue card, or just your Medicare number.
A list of your prescriptionsDrug names, dosages, and how often you take them.
Your doctors' namesPrimary care and any specialists you see regularly.
Your preferred pharmacyWhere you fill prescriptions now, or would like to.
Your current insurance cardIf you already have a Medicare plan, bring the card.
Your Annual Notice of Change letterIf you received one this fall, bring it along.
Any upcoming procedures or visitsHospital stays, surgeries or specialist visits you expect this year.
Your questionsWrite them down beforehand — nothing is too small to ask.
